Dear Referring Physicians:

• Archer Radiology specializes in providing medical professional diagnostic imaging services to hospitals and clinics as well as state-of-the-art services in its own outpatient imaging centers. 
• To refer a patient to Archer Radiology, submit a referring physician form with your signature. 
• If you are scheduling an exam that requires a contrast injection, a recent creatinine may be required prior to the exam. For certain procedures, some insurance companies require pre-authorization. 
• To request a patient's medical records, submit a medical record request form with the patient's authorizing signature.
