CT Upper Extremity - Benefits

CT of the Upper Extremity - Positive Findings

Tumors, benign, malignant, metastatic

• Suspected primary or metastatic bone or soft tissue tumor by preliminary imaging or significant abnormality of physical examination; or
• Follow up of primary bone tumor to evaluate lesion density and adjacent bone.

Infection or Inflammatory Conditions (known or suspected)

• For diagnosis of osteomyelitis, septic arthritis or abscess; or
• Location of bone sequestra.

Fracture or Trauma Evaluation when adequate diagnostic evaluation is not available on plain films

• To evaluate the anatomy of fractures and subluxations; or
• To define the extent of an acute fracture and position of fracture fragments; or
• For preoperative evaluation, especially for complex fractures of the distal humerus; or
• To assess fracture healing, for callus formation and solid bony union; or
• To confirm a suspected (occult) fracture following initial radiographs.

Neuropathic Osteodystrophy (e.g., Charcot Joint)

• To evaluate suspected complications (e.g., infection) when plain films are not diagnostic.

Other signs, symptoms and conditions

• Hemarthrosis documented by arthrocentesis, if MRI is contraindicated; or
• Osteonecrosis, if MRI is contraindicated; or
• Intra-articular loose body, including synovial osteochondromatosis; or
• Suspect intra-articular osteocartilaginous body of the elbow; or
• Significant persistent pain unresponsive to a trial of 4 weeks of conservative management following initial assessment with conventional radiographs; or
• Abnormalities on other imaging (plain films or bone scans) requiring additional information to direct treatment decisions; or
• Accompanying an arthrogram (CT arthrography); or
• To evaluate palpable mass on physical exam; or
• When additional criteria for MRI are met (see below), but MRI is contraindicated.
